It's an OOTP bug with the SQL dump for the players_contract sql file. The players_contract_extension table is fine.

None that I know of.

I have three and installed three statslab apps with each set to default to its league.
As Mark says, you can have separate installs for different leagues. You define the league ID of each install on the admin page of StatsLab. If these leagues are all within the same saved game, then you can point them all to the same database.
