I kind of do a mixed bag depending on my mood and where I am at with the franchise I am running.

Opening few weeks of the season, big games and playoffs I'll usually watch my games on an inning-by-inning basis. I let the CPU manage the in-game situations. The only time I actually watch the play-by-play is late-innings in important games when it is close to really ratchet up the drama.

I do manually manage the rosters, rotations, line-ups and depth charts unless my team is terrible and I am trying to rebuild. In that case, I'll make sure the key guys I am building around get playing time but outside of that, I'll let the CPU decide then sim on a week-by-week basis to try and accelerate the game.

One of the things I love about the game is that you can really be flexible with how in-depth you get. I can get an entire season in while waiting at the doctors office or I can let the suspense of a single World Series linger a week or more as I get a moment here or there during my day to watch an inning of play-by-play drama.
