I've had a pitcher become exhausted after a really long rain delay. I immediately remove him for another pitcher in fear that i may be risking injury and/or have him really struggle to get outs. Unfortunately, when this has happened, i typically don't have another pitcher warming up in bullpen so they come in cold (I do also warm up a second relief pitcher in the meantime so if the guy i brought in struggles, I have another guy ready two batters later)...
I have a similar question though actually...when i am playing out games and my SP gets injured early in the game and has to come out mid-inning...does the relief pitcher i bring in come in "cold" since he hasn't warmed up?...or is it like real life where he has as much time as needed to warm up on field, so he isn't "cold"?
