My answer is based on just one instance today, but... I had my pitcher in a real mess: Nobody out, bases loaded. I made the mound visit and wasn't even thinking about taking him out. After the visit he got a double play and a flyout and got out of the inning. So to me, visiting the mound is always an option from now on.
