I like to make a fictional league with teams for a local flavor. I have more interest in the league this way. Since I live in Connecticut I have a New England league where all players are from the same region. Other players like state leagues or country leagues, even fictional places like Game of Thrones or Star Wars. In OOTP you can use your imagination.
