I think if you are playing the same game, it won't update the names database until you install the new files and use the "reimport names and ethnicity database" function.
As for patches, it seems to overwrite these files so I temporarily move them to a new directory, then move them back after the update is complete.
Anyone reading this is free to correct me if I have this wrong.
