Can anyone explain? What did I do or what happened? I've been playing OOTP for nearly 20 years (OOTP 4) and never seen this before.

I should add that I noticed when I got a message that I had an invalid lineup. I then enabled commish mode and added him back to the Phillies via the Editor.
