This will be helpful, but this isn't just an issue with fatigue. The AI will leave the pitcher in to give up an unlimited number of runs if you're clicking the sim until end of inning button. It doesn't use any pitcher management strategy. If you keep clicking that button for multiple innings, the AI will not remove the pitcher, even if the starter gives up 8 or 10 runs. In every instance I've seen, it will only change the pitcher once exhaustion is reached.
