I have ghost players enabled. Does that mean that even though a MiL team has 15 players it actually has a full roster and that I don't have to do anything to manage the roster size?

Correct. The ghost players will fill any lineup/pitching spots that the real players don't.

So it seems using GHOST players is better so that you don't mix up fictional and real player in a list view.

That is the real purpose for ghost players. People want a complete minor league structure for historical and, without using fictional players, there aren’t enough players to fill the leagues. With real minor leagues enabled, the problem is solved with actual players but that feature was only introduced a few years ago.

But if you exand a real league you will eventually run out of players
