Got it for you. (I've seen this about 3 times now)

Men on first and second. You can see in the attached screenshot that there was only 1 out. Then the PbP shows that the inning ends without description.

I did confirm in the Game Log that it was a 1-6-3 double play. You can also see that "two runners left on..." is incorrect. Should only be 1.
