I can't speak to your phantom ER. But the second glitch you saw is probably this issue:

Basically, there is no PbP text for a 1-6-3 DP. For now I'm treating it like the play-by-play announcer's mike cut-out temporarily. Hmmm. . . wonder if it does that on Bonds homeruns?
