I am no expert, but I think you have to work your way up to the majors. I have found that if you play one season as a minor manager, regardless of your performance, you should get some offers the day the playoffs end.
It is also possible that you could accept a minor position before the end of the MLB playoffs and then get offered a MLB GM position after the playoffs, but I have never looked.
You can also set yourself as commish and take a position that way.
