This has happened to me on several occassions... not sure if its purely AI team related. But I had 2 human teams submit a trade for a player on a AI team... both teams reported that they had the AI GM saying "good deal". upon export and the trade being processed... no mention of the trade was listed anywhere... some players made it to their new team... some didnt... some were released....
so far no more problems have happened when I restricted teams from making trade proposals through exports

The trades were with human teams, and no trades were processed in the game. They were all processed by the commish.
