Depending on the level of detail you want, you could download logos, stadium photos, player photos...for a long time I just played with the players imported from Lahman without any further kind of customization. After a while I grabbed some of the excellent logos board members had made, then WrightWing's Player Photo set which was particularly cool if you're starting a century or so ago. It really adds personality to the players you've never heard of, makes you see them as human beings and not just a series of stats. This is the first version I'm using ballpark photos for, in a league starting in 1901, and it's beyond cool for the same reasons. When you have a backdrop of a black and white ballpark with the little circular photos of the players out on the field it's as close to realistic as a text sim can get.

So it all depends on what level of immersion you're looking for. If you manage every game like I do, grab someone's player photoset, logos, and ballpark photos. If you sim over a long period of time and are looking just to compare stats, the photo sets probably won't add too much to your experience. Either way, you can start playing right away with the Lahman database. Play for a while and see if you think you'd have more fun with more photographic detail.
