Bit different depending on the OS you're using.

In Windows XP, right click on the shortcut (either on desktop or in Start Menu) and click on Properties. Click on the Shortcut tab. Add nofacegen to the very end of the Target line, after the last " in that line. OOTP will then run with Facegen disabled.

Other Windows OSes are similar to that.
