I've seen this done in other leagues. In the league I run, I do charge teams $250k for missing 3 sims in a row and don't return the money even if I replace the GM. For a team with $20M in the bank, this isn't a big deal. For teams who are in the red, it makes their situation worse, and in that case I would consider returning the fine.

Thanks for suggesting it. I may change my policy in my league.
