Here is an idea you can try. I'm listing everything, even though I'm sure you know. Just covering everything.

Go \OOTP Baseball 11\data\saved_games\*YOUR LEAGUE NAME HERE*\news\html\images

Find the Team that keeps changing and delete all the logos for it.

In the logo folder, OOTP Baseball 11\data\logos, have the team logo there.

For instances, if the team is Gotham Knights, image should be called gotham_knights_000000_ffffff.png

Load the league up and see if the game is pulling the right image. If not, check the spelling. Could be a slight misspelling somewhere either at the image or in game. Try erasing the name in the game and retype it in.

Also, is this happening in a league that was imported? Or a team that was renamed at some point?
