Something similar to this happened occasionally for me with OOTP 12. It was a weird black parasite in a player face and like your problem same or new picture did nothing to remove it. What I had to do to remove it was just to import a different .fg files, after everything was back to normal.
Maybe that could work for this too, we never know...
