Plays where a runner tries to score from third are considered a difficult play and an outfielder will never be penalized for failing to throw someone out at home, even if he airmails the throw. Here the error is appropriate because it allowed the other runner to advance a base. However, if the runner on third was the only man on, there could never be an error on such a play. Similarly, that runner scoring from third will be an earned run (and the batter will be credited with a sacrifice fly) regardless of what happens with the throw from the outfield.

I've said this for several versions of OOTP now, and I'll say it again: there needs to be someone on the beta team who is an expert on scoring games. These sorts of errors are all too common in the game, and they have been for a long, long time. I may keep mentioning some of them from time to time, but trust me - for every one of these I mention, I notice and don't mention maybe 50 of them.
I appreciate you pointing these scoring mistakes out. I notice them as well, and they irk me a bit. Once an error is charged in an inning it's like the game will take every opportunity it possibly can to not charge the pitcher with any earned runs. Even when they clearly would be in real life.
