Option 1 leave him in
Option 2 pull him for player being warmed up
Pitcher says "I am tired, it's your call"
Option 1 pull him
Option 2 tell him that this is his last batter so he doesn't have to leave anything in the tank
I play out my games and would love to see this area of the game expanded.

If you get either of these messages, I would pull him if there is anyone on base; if bases are empty, let him finish out.
I usually visit the mound to break up the pattern. Most times it works and something different happens (not always good; for example my pitcher has walked back to back batters - I go out, and he gives up a 3 run homer), but if my pitcher is tired, unless he's working on a shutout, he's coming out.
