I do believe that we figured out that it was something to do with Away uniforms. My workaround has been to set it to update every Jan. 1, and then go into the "Edit Colors & Uniforms" screen for every team and click "Update this team's pictures NOW."
This does mean that any time a player changes teams, he'll be "stuck" in his old team's uniform, but I've gotten used to just force-updating the opposing team's pictures before every series I play. It's not a huge deal for me, but I'm surprised this was never fixed.
