2) All the names that don't belong seem to be coming from the US (Historic) nameset. If you're using the default files then there's a good chance you'd never notice this (except on the occasional "Mac Ortiz"), but with Questdog's names seeing a white guy named Kareem O'Malley or DaQuan Zimmerman really stands out. 
3) Exiting the game appears to fix the problem. So the (hopefully temporary) workaround is: create your new game. Exit OOTP. Re-launch OOTP. Delete all players and personnel, and create new ones. |
Are you by chance playing the MLB quickstart?
Questdog's name set is intended to be used with fictional setups. It does not get along well with the MLB quickstart.
