If you're on a PC the path is basically

C:\Users\User\Documents\Out of the Park Developments\OOTP Baseball 16\saved_games\YOURLEAGUE.lg\news\html\images

Go the player folders pictured below, person pictures and profile pictures, and empty their contents, but of course leave the folders intact. Backup the league, if you have any doubts or worries. There's a file called faces.dat in your league file and that should regenerate all of your original faces, so you don't lose the ones you have now, and generate new facegens for those 'on demand' as your setting indicates.
