So I thiiink I found the solution for my league.
In the past, I had the correctly-named files in the main OOTP jersey/ballcap directories so when I pressed "auto-load" for a team's jersey/cap it loaded them just fine and copied the files to the league's jersey/ballpark folders correctly. However, when pictures for players were generated, some jerseys would revert back to a OOTP default based on team colors, even if I had the "lock" option checked.
I eventually noticed that after I auto-loaded the custom art that some of the "jersey type" drop downs still said "Standard" instead of "Custom Texture". And it looked like THOSE were the jersey/caps getting overwritten with defaults.
So the trick I'm using now is to copy the correct art into the league's Jersey/Ballcap folders and then go into each teams Settings and Colors page and making sure ALL the drop downs read "Custom Texture" and that the lock option is checked. Kind of a pain, but it's worked through two rounds of generating player portraits.
