My best guess is that you erred on the custom playoffs screen.

If this is a one-division setup, don't bother with wild cards, and just advance the top however-many teams from the division.
If this is not a one-division setup, you probably chose to advance the third and fourth place wild cards instead of the third and fourth place teams.
