John Hudek made three MLB stops in his final major league season, 1999,
beginning the year with the Reds and winding up with the Blue Jays. In between, from April 16 to July 30, he was a member of the Braves. He pitched in 15 games (0-1, 6.48). He was acquired by the Braves for former closer and World Series hero ('95) and goat ('96) Mark Wohlers.
Found this in the third edition of the Fan! program and cleaned up the background. Literally the only one I've seen, but I'd expect more are out there to be found.
