Tom Dougherty 1904
Dougherty pitched on April 24, 1904 in relief for the Chicago White Sox. He retired every batter he faced in the 2 innings he was on the mound. He was featured quite prominently in the team photo (which I have attached). And yet, he never got another opportunity in the majors.
