Go to the User Control Panel and Edit Signature. Make sure that there are no other spaces or characters saved there, especially leading characters before the [IMG] tag. Put your cursor behind the final image tag and hit the delete key several times. Save signature.
Also go to Edit Options and be sure that "show image..." is checked under thread display options.
