A couple of questions
(1) When you did you start this file?
And (2) Are you on the latest hotfix?
We addressed these issues with it, otherwise we're going to need to know exactly what you're doing because we haven't seen many issues since. The problem before the Hotfix was game files were being corrupted, but as far as I know that hasn't happened on the new build.
