This has nothing to do with your settings.
When in a starting role, even at low stamina, a pitcher is going to be able to get around 60 pitches in. This, for the most part, makes sense. There isn't a major league pitcher who would not be able to throw that many pitches in a game if they were "stretched out".
What the game does not do a good job of simulating is whether a pitcher is "stretched out". If they are the starting pitcher (but not in an Opener role) the game just assumes they are stretched out and they get that benefit.
