I'm not that technical; but, with some direction would be happy to share world and name files.
I changed the #21 (Albanian) names to match the noble house names of westeros, and included common medieval professions (Baker, smith, hostler) as well as some more common region appropriate names (Rose, Winterly, Summers, Ironborn, Fingers, Kingswood). I use the nation of Bermuda for Westeros, and added cities and states (9).
The most time consuming thing is modifying the birth city or last name after player creation/before the draft.
I don't user minor leagues or DH. For the non-relegation league, I have 8 divisions of 7 teams (& took liberties moving some teams slightly between regions - especially the Iron Isles - I combined the Stormlands and Crownlands). I considered going with 7 Divisions, but, I didn't like the way the teams were breaking down/had trouble deciding who to cut in some cases.
I'm finishing up the uniforms for the promotion/relegation version now (I had issues copying world file, and had to re-create). 3 Leagues (Premier Lords, High Lords, Lesser Bannermen). Each league has 8 divisions of 4 teams, for 96 total teams (again, combined Crownlands and Stormlands).
Then I basically just use the Seats of Power/stronghold of the noble houses as cities, or a nearby river in some cases, and house ancestral swords/words or specific buildings/locations where I can for stadium names.

Winterfell Direwolves - Ice Park
Weeping Water Flayers - The Dreadfort
White Harbor Mermen - Fishfoot Yard
Barrowton Blades - Barrow Hall
Example (Dorne Premier Lords):

Skyreach Blue Hawks - The Soaring Towers
