A correction: The Kansas City manager who out-strategized Bobby Cox in the 1985 ALCS was Dick Howser. Jim Frey was the man who took KC to the 1980 World Series (beating Howser's Yankees in the ALCS). My bad.

Frey also managed the 1984 Cubs. You can argue about having Leon Durham at 1B in the deciding NLCS Game 5, but the ball went through his legs in the 7th inning, not the 9th, so I don't know if it was a pure Buckner moment. (Cubs only led 3-2; perhaps Frey thought they might still need Leon's bat.)

Great Moments in Executive Thinking (a series):

1979: "[Nolan] Ryan only went 16-14; we can sign two guys who go 8-7 and it will be the same thing." —Buzzie Bavasi, Angels GM

Ryan leads the Astros to the NL West pennant; the Angels collapse.

1987: "The sun will rise, the sun will set, and I'll have lunch." -Red Sox GM Lou Gorman, being untroubled by Roger Clemens leaving spring training in a contract dispute.

Clemens takes his time returning (and eventually signing), the Sox start Bob Stanley on opening day, never have a chance to defend their A.L. title, and don't see the Series again for 18 years.

2019: "We're not going to pay [Zach Wheeler] for two half-seasons." —Mets GM Brodie van Wagenen.

Zach signs with the division rival Phillies, currently leading them to 1st place. Mets seem unlikely to repeat the 86-76 record of 2019 any time soon.
