Primarily by winning, but there are a few storylines that can randomly increase it as well.
Fan loyalty affects a number of things, but generally as a smaller contribution to part of a larger formula: attendance (both at home and, to a much lesser extent, on the road), players joining European youth teams (owned by a parent team), the frequency and amount of fan happiness changes, the tendency of the AI to choose special roster management strategies (Win Now vs. Rebuild), and a few more things I can't recall at the moment.
