From a combo of Hockey Reference game logs and my old copy of Total NHL (circa 2003) , I can offer the following (surnames A-C only so far):

Maxim Afinogenov, Buffalo 02-03 , Head Injury in camp (8-10-02) , missed first 46 games.
Ron Anderson (21-1-1950), Washington 75-76, Knee injury in camp, never played then traded to New Haven AHL (26-2-76).
Donald Audette, Buffalo 95-96, Knee injury in camp (23-9-95), missed first 11 games.
Frank Bathe, Flyers 83-84, Back injury in camp (15-10-83), missed entire regular season, returned for 1 playoff game then retired.
Jean Beliveau, Montreal 61-62, knee injury in exhibition vs Spokane (30-9-61), missed first 25 games.
Doug Bentley, Chicago 44-45, sat out entire season to work on family farm to help war effort (was alternative to military, so maybe shouldn't count).
Drake Berehowsky, Phoenix 02-03, knee injury in camp (24-9-02), missed first 61 games.
Todd Bergen, Flyers 85-86, suspended for not coming to camp and became a pro golfer, traded to Minnesota (29-11-85) - never played again.
Blair Betts , Calgary 02-03, had pre-season shoulder and knee surgery (1-10-02), missed first 72 games.
Wayne Bianchin, Pittsburgh 74-75, neck surgery (April 74), missed start of season (24 games), returned for 2 games, then out for season.
Phillipe Boucher, LA Kings 99-00 & 00-01, Foot injury in camp, missed first 80 games (returned 7-4-00). Then missed first 60 games of 00-01 (returned 22-02-01).
Ken Breitenbach, Buffalo 77-78, missed entire season after breaking leg in camp.
Keith Brown, Chicago 92-93, Shoulder surgery (27-9-92), missed first 47 games.
Randy Burridge, Washington 92-93, knee injury in camp (5-9-92), missed first 80 games.
Billy Cameron, Canadiens 1924-25, suspended entire season after being traded to Vancouver (WCHL) but refusing to report.
George Carey , Montreal Wanderers, 1917-18 and 1918-19. Traded by Quebec to Montreal in 1917 NHL dispersal draft but refused to report. Was suspended
by Montreal for 2 seasons until traded back to Quebec when they joined NHL in 1919-20. Sources tend to only show his NHL career beginning in 19-20.
Jack Carlson, Minnesota 79-80, Missed entire season due to back surgery.
Kent Carlson, St Louis 86-87, missed entire season after spinal fusion surgery.
Gerry Carson, Canadiens 35-36, missed entire season after knee surgery.
Bruce Cassidy, Chicago 84-85, missed entire NHL season due to knee injury in camp, but returned to play 27 games for Ottawa (AHL).
Shawn Chambers, Washington 91-92, missed first 47 games due to knee injury sustained while with Minnesota (5-12-90).
Dick Cherry, Boston, retired from 1963 to 1965 to work as a teacher. Mostly in AHL with Boston, but later played for Flyers.
Marian Cisar, Nashville 01-02, hip injury in camp, missed first 4 games, then played in 10 games, then out for season (2 games in AHL).
Charlie Conacher , Toronto 36-37, Wrist injury in camp, missed first 2 games, then played off and on for 3 months (15 games total).
Pat Conacher, NY Rangers 80-81, ankle injury in pre-season rookie game, missed entire season.
Troy Crowder, Detroit 92-93 & 93-94, injured back 10-10-91, missed most of that season , then entire 92-93 and 93-94 seasons.
Mike Crowley, Minnesota 01-02, achilles tendon injury in camp, returned to play 11 games in Houston (AHL) that season then retired.
Glen Currie, LA Kings 85-86, back injury in camp, missed first 67 games.
Paul Cyr, NY Rangers 89-90, played first game of 88-89 season then had back surgery. Missed rest of season and entire 89-90 season.
