If you go to the "Structure" tab in "Edit League", you'll find that the series length for the Round of 16 is blank. If you put in a Series length, the playoffs should work normally. Seems like to me the game is forgetting to put in a series length for the extra round at set-up. I've had the same issues, and manually putting in a series length has worked for me.
