I don't think I was using the warmup rule. But maybe you do have to drag the reliever to the bullpen before inserting. I'm going to go try that.

Here is the first issue I run into. As you can see Ohtani is the scheduled SP for the upcoming game and is also the DH in the daily lineups for the upcoming game.
