While playing out entire seasons, I sometimes use the sim entire inning to get through a couple of innings on defense, but previously this meant your pitcher would remain in the game without any warning until reaching exhaustion, regardless of how many runs were given up. Thankfully, there is now a limit and warning based on performance, although I would love to see it at 3 runs instead of 4. Even better, it would be great to have the option to set the preferred limit, especially in cases where it's a reliever and not a starter. Late in a game, the warning should be be quicker with relievers, since a manager is going to be quicker to hook them. But I'm happy to see a big step in the right direction here.
There are some cases it can trigger earlier with a reliever, but yes, it's definitely been requested and I think we've all been burned in the past by simming a half inning and suddenly you're losing 12-6 and your pitcher threw 76 pitches in the half inning...

It's always a balance between kicking you out too early, vs too late. I think if it's that close that 3 runs would make the difference, you really should not be simming the half inning, and should be doing RISP instead almost always. Where we have it should catch you before things get way too out of hand most of the time, or also kick out out if you aren't paying attention and click one too many times.
