I have been experiencing the same issue since I traded for Ohtani at the start of the 2023 season. (Thanks to kq76 for highlighting this thread already running.) Here goes.

Looking at the screen shots in previous posts I saw a similarity with my own set up. I think the reason why some of us have a problem and others do not depends on whether you have imported an earlier version of the game into OOTP24. I have been running my game for a few years and in those days Ohtani occasionally played RF (as well as DH). The simulation does not seem to be able to cope with a 2-way player who can also play a position. This is what I did to solve the problem.

1) Enable the 'SP can remain as DH when removed' in Game settings/Rules.
2) Make sure Ohtani is set as a 2-way player and check 'Skip DH when pitching' in his personal game strategy page.
3) In the player editor for Ohtani, make sure his experience for any outfield position is 0. This will mean he is now listed as a DH (rather than, say RF.)
4) I use 'Daily Lineups' (play each game, each ball) so place Ohtani in the batting lineup as DH but leave the SP blank (the game will pick Ohtani up as the pitcher from your starting rotation).
5) When you start the game, Ohtani should now be your SP and DH.
6) When you want to take Ohtani out as SP, use the Bullpen widget on the main screen (stadium view) to replace him with an RP. Ohtani will stay on the batting lineup and his position will change to DH.

This worked for me.

It may be possible to do things differently at steps 4 to 6 (above) but steps 1 to 3 are vital. I will play around with this the next time his spot in the rotation comes up.

This does mean that unless the programming can be changed to overcome this issue, Ohtani cannot play in the outfield (not that he really does anymore, to be fair.)
