For 1 and 3 - yes, you should be able to accomplish this using the associations feature. For 2 - theoretically this is possible but honestly it would end up being a bit of a headache to set up. If you set your league's region to North Carolina, then you can at least have it so that all of your American players come from North Carolina, and assign the percentages you want to the non-American players from other countries - but if you want certain proportions of NC players to other American players, you'd probably have to use feeder leagues - one feeder league for NC players, one for surrounding states, and one for the rest of the country, and then get the ratios right.
