In terms of general advice, if you're playing the game for the first time a lot of the default settings will be a challenge. Easiest way to keep it challenging is to keep commissioner mode off and not edit any players/contracts. Though if you want more then that, the best way is to "role play" as a GM. Sure, you can min/max in trading, but focus more on balanced deals that make sense for both teams, don't claim guys off waivers or sign players immediately just to trade for prospects. Kind of police yourself
To the specific question, yes, you can do a live start first. Once the game starts, you'll have access to all the settings, and adjust the trade and injury settings. Personally I play with the High/Realistic Modern day injuries and its a fun challenge. Really forces you to build depth and not just rely on a few superstars to carry you
