It's really not that uncommon in the majors. I doubt this is a bug, you may just only be remembering times it happens or just some weird RNG that strung these events together. But no, every team and player doesn't always prevent the trail runner from going to second

In my games they do. My biggest complaint about it is there is no variation on the play. The runner always reaches 3rd and the trail runner always reaches 2nd. If you are telling me Arizona is going to challenge Brenton Doyle's (75 arm) with Ivan Melendez (Speed 35, Base running 30) with a turtle like Justin Turner as the trai runner in a 1-0 game in the 6th inning and it is successful 100% of the time then it is a bug.
