I've only seen this happen at one game I've been to. Never had it happen in an OOTP game I've played out until now.

Has anyone ever seen 5 or more? IIRC that's happen in a real life HS game.

I've only seen it once in a game I played. Can't remember who it was, though. Some no-name reliever, I think.

I've had this happen before; a couple of times legitmately, but a few times it was because I was trying to make my guy a true utility player, and catcher was the last position he needed to learn.

I've seen it before in OOTP16 don't remember seeing it in 15 though.

I was also a part of a walk off strikeout game winner back in the days of little league. Granted back then it happened a lot.
