Same pitcher picked 3 games in a row

My team was in the division championship, and I was manually changing my starting pitchers for each game, and rearranging the order. I had to leave for the rest of the day and set my starting pitching line up and kept it at strict order.

For some reason, the game picked the same pitcher, Jose Barrios, for 3 games in a row, and he is still showing as the pitcher for the next game (even though I lost the division)
