How do you prevent players that only played in minors from making it to majors?

If you are referring to a historical game with historical minors & minor-leaguers, I'm not aware of a way that you can do this. While I can think of instances where this might be desired, I can also think of where it would be impractical. If, say, in real life a major league team only used two catchers all season long, but in the sim both of those catchers got injured, the team would be pretty likely to call up one of its real-life minor league catchers.

If we're talking about the same thing (which I think are) you can click on "Game" at the top, and then click on "Game Settings" at the bottom of the list. When it takes you to the Game Settings, click on the "PLAYERS & FACEGEN" tab. All the way on the far left and all the way at the bottom, you'll see a section labeled "STATS SETTINGS." Right above that, literally the first thing you'll see is the option that says "Delete those who never reached the Majors." Just click that box and you're good to go.

While I've never used this, I don't think it's what the OP is asking. Rather, this setting refers to retired players, and if they never reached the majors then they are deleted. If I'm understanding, I would assume this would be a housekeeping feature to save space, but has no effect on whether career minor-leaguers can reach the majors in OOTP.
