Preface: I play historical...
I completely turn off long-term injuries, because they are no fun. I have players retire according to history to replicate actual career-ending injuries. I have short-term injuries at OOTP Classic. |

Pretty much the same here. When doing early 1900's historical, especially if playing with minors, I set injuries to low or even very low (so teams don't run out of players). And then to compensate for that - and not have lots of guys playing 154 games - I set position player fatigue to high or very high.

High realistic for all players/situations.
FWIW I manage every inning of every game.

I play random debut. Once I have enough players, I use high(modern day) for short term injuries and either OOTP Classic or High(modern day) for long term injuries. I don't like looking at career stats for gobs and gobs of players that never missed time for anything other than fatigue.
