It's the female equivalent of Randy "Big Unit" Johnson.

Let's make up a backstory
First time in the minors, guys take him out as it's 18 birthday, and they are near the Canadian border...kid grew up in a religious household, so he didn't know what a strip club was. The guys and he come in between dancers and it's dark so he can't see what's go on around him, lights come on and a woman with Size EEE comes stumbling on the stage and Mike blurts out "TIT!" and that is all he says for the rest of the night Nickname is born |

I prefer a background story that has something to do with a mishap working on a farm as a kid. Fill in your own specific details as you see fit.

Well, Tug McGraw got his nickname from his mom because of the aggressive way he breast-fed as a baby, so maybe it's not so far-fetched.

According to baseball reference, Charles H. Willis is the real player with that nickname. He was a minor league player in the 1880s and 1890s. I kinda wish I could have found more info on him. :/
