Pitchers still in the field?

I was playing out a spring training game, and an inning after replacing my pitcher, I was reading the play by play, and it mentioned that my previous pitcher took the relay throw. It did not show him on the field at any of the positions though.

I apologize for the delay in responding to your question. Have you seen this problem reoccur at all? Is there any chance that there were multiple players with the same name?
