I'm not sure I understand the AI associated with Perfect Team. Can someone tell me how a team with Diamonds and above only wins .250 of their games? Does this happen when you don't tinker with your team after each season?

The OVR on cards is not in any way related to the quality of a team. Additionally, if you use it (as some newer players do) the AI auto-roster-generator really does not understand what makes a player good or suitable for the different defensive roles. I would say "wow, so you can't just stack any old diamonds on a team and win?" is the first realization new players make once they start to really understand how the game works.
