Starters getting skipped?

I usually run my team with strict rotation.
However, last season, I had a starter get started 3 times in 6 days. He'd get blasted in his 2nd and third starts and pulled early, and would start again a day or two later, and then get blasted again - even with strict rotation. It made an absolute mess of my bullpen and sent me into a tailspin.

I just had this happen again - despite a strict 5 man rotation, Jake Peavy started 3 times- 4/10, 4/13, 4/16.

I do not use 7-day lineups. This seems to occur after I've made substitution in the rotation - I just swapped out one pitcher for another recently- but not Peavy - his spot remained the same. Any insight into this?
