Extra-inning snafu--pinch hitting

I'm playing Atlanta against the computer (Baltimore) in what should have been an epic game--battle between first and second place teams, headed into extra innings. Every time Atlanta scores, Baltimore answers back in the bottom of the inning.
<p>In the fifteenth inning, Baltimore is on their final relief pitcher, Frank Fields (who ends up with 12 strikeouts in his five innings of work!) Congratulations to Frank, but he had some help when, after Donald Sinha pinch-hit for him in the bottom of the 15th, he came back out to pitch again. The computer obviously felt guilty, since at the start of each inning it would say "Donald Sinha is the new <blank>." But not so guilty that Sinha didn't pinch hit again for Fields, in the 18th. His double darned near cost me the game. But we won, so I file this error report with a smile.
<p>Does anyone want to see the box score or log of the game? Or is this bug already well known? I seem to remember it occurring in version 4, but I am using 5.11.
<p>Something I can't show you, but also interesting, is that at the end of the game, the PbP says "Atlanta destroys Baltimore 9-8." Funny, because Baltimore out-hit us, 23-14. I had thought that the "so-and-so destroys such-and-such" was a function of the relative number of hits--I have come to enjoy reading "Atlanta destroys Miami 4-3" when we have out-hit them 13 to 4--but this was the first time it has shown up on a stark reversal of fortune such as this.

Okay, so now I'm reduced to replying to myself because, a few games later, I have a related issue.
<p>We (Atlanta) are playing Miami. Miami has run out of position players, so when they get to the 9 slot, the relief pitcher, Casto, is forced to hit. Later, their 3 hole hitter and center fielder, Esteban Coronado, is injured legging out a double. It's an ankle sprain. But because there are no more position players, and the game doesn't seem to like to pinch run pitchers, he stays in the game.
<p>Okay, this isn't the end of the world. It's only an ankle sprain, and he shows at 93% effectiveness. I realize that this is not why he stays in the game--he stays in the game because the AI has trouble when all players are used up--but I can live with it.
<p>So here's the weird part. Later, Casto, the pitcher is up again. The computer says that there is a pinch hitter--Esteban Coronado, the guy with the sprained ankle who stayed in the game. The line-up is still showing Casto but I think that the field is showing Coronado in the batter's box (not 100% sure about this last). Whoever is hitting makes the final out. When I check the box score, I see only a single at bat for Casto, but what looks like an extra at bat for Coronado. In the log of the game, Casto is shown as making the last out!

And again: 16th inning, versus Philadelphia. Their pitcher--their last relief pitcher, Hugo Couch, is up for the second time. I mention this because this duplicates what happened in my first post in this thread: The last relief pitcher is in the game. The first time he is up, he hits. the second time, someone pinch hits for him (2B Hector Carillo). Then, in the bottom of the 16th, Couch is announced again as the new pitcher. After the batter is announced, Carillo is announced as the new 2B, although there is no evidence he really entered the game.
<p>Carillo got a hit as the PH, which could have cost me the game . But, we got out of it, and won again . I suppose that one of the reasons is that I, the human, was able to break down and bring in a starting pitcher, but the AI was stuck with tired (and illegal!) Hugo Couch.
<p>If you play out the games, you will have a few long games every season. This bug takes some of the fun out of what should be among the more memorable occasions in a season.

Similar situation here...

Played out 2003 as the Mets using the latest Total Minors roster...after a horrendous spring and summer we were respectable in the fall and I had high hopes for 2004.

SO!

Season opener vs Houston, the game goes long, 15 innings. I am much more conservative about pulling my pitchers, so I still have a MR or two left but Houston has Wagner on the mound. He goes red, and I'm thinking - woohoo! I can start hitting this guy no problem now and win the game....!

Well, he gets out of the inning, the Astros pinch hit for him in the top of the next inning, and when I come out to bat in the bottom of the 15th, there he still is - on the mound! What's WORSE, he is VERY fatigued and STILL set em down in order.

Naturally, when Houston comes up again they score two runs and I get so perturbed that I quit the game and start over.

Any chance of a fix?

Hey Steve ---

In the grand scheme of things, the bug is relatively minor since I could just restart the game and (I think) there was no harm, no foul. FYI, I am using OOTP 5.12.

To further clarify, Houston had used all of their relievers at that point; however, they still had 2 rested starters. I tried to make the PH pitch, with no luck; also tried to put one of the starters in to pitch for the 'Stros, but wouldn't let me.

That's about al I can recall
